The Wellfamily Service is a well established service that has been supporting patients registered at 7 GP practices for 20 years. The service works with patients with complex, often long term. problems who present to their GP with physical symptoms associated with social and psychological worries. The service works with patients of all ages ‘from cradle to grave’ and offers a range of interventions.

You will have a relevant qualification and at least two years experience of working with adults, young people and/or children and families in a community setting. You will have experience of carrying out holistic assessments to identify needs and goals and, if appropriate, be able to work with the whole family. The service works with people of ages so you will be confident in working with older people, adults, young people and children. You will have a good understanding of the many issues that impact on emotional health and wellbeing such as debt, housing, bereavement, family breakdown, domestic abuse and substance misuse and a good knowledge of local specialist services. You will be non-judgemental and be able to build trust with people of all ages and from all backgrounds.
You will work closely with your colleague in the Wellfamily Team and with the new Social Prescribing service to promote both services to our 7 GP practices (3 in Swaffham, Boughton, Feltwell, Litcham and Great Massingham) and ensure that patients are referred to the appropriate services. The team will triage all referrals within 2 weeks to reduce the waiting time for services and you will carry out initial holistic assessments of those referred to the Wellfamily Service. You will then develop an action plan with interventions and goals agreed with the patient and review regularly. You will work with local partners to benefit patients but also to find opportunities to work together to maximize resources. You will ensure that casework recording is accurate and completed within timescales and that evidence and outcomes are routinely captured and recorded for reporting to funders. You will undertake group work as appropriate and work with the team to be creative and flexible in considering how to meet identified needs.
