Living Well Connector

Family ActionHours: 18.5 hours a week

Salary: Grade B, point 10 (£19,086 pro rata)

We are looking to recruit a Living Well Connector to help improve people’s wellbeing.

Living Well Connectors are part of the development of Social Prescribing across Norfolk. Social Prescribing aims to address non-medical issues that may be causing or exacerbating health problems (e.g. debt, poor housing, mental ill health, social isolation, etc).

In the Breckland and King's Lynn and West Norfolk District areas, Social Prescribing is being delivered by a partnership of Age UK Norfolk, Community Action Norfolk, Family Action Swaffham, Future Projects, Home Group, The Matthew Project, West Norfolk Carers and West Norfolk Mind.

Family Action is recruiting a Living Well Connector working a minimum of 18.5hrs a week to work with the 3 GP Practices in Swaffham, Feltwell and Boughton and their satellite surgeries.

The Living Well Connector will advise and support individuals on a 1:1 basis, with the aim that they will experience improved health and wellbeing. Some of that support will be delivered directly by the Living Well Connector as well as through linking the client with a wide range of specialist services and support in the community. The service is intended to be time limited support, which encourages self-service and empowers clients to take charge of their next steps.

Applicants must have

  • a relevant Level 3 qualification in health or social care
  • experience of working with individuals on a 1-1 basis in a voluntary, community or health sector in a voluntary or paid capacity,
  • an understanding of the wider determinants of health, including social, economic and environmental factors impacting on emotional health and wellbeing. A proven ability to interact with people in a way that inspires trust and confidence and an ability to motivate and influence others
  • A good knowledge of voluntary and community services in Norfolk, how to find out about local services and how to support people to access them.
  • Experience of collecting data, monitoring, evaluation and reporting.
